📌 เงื่อนไขการรับบริการ
1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำ E-Coupon มาแสดงเท่านั้น
2. ในกรณีที่ไม่นำ E-Coupon มาแสดง ท่านต้องชำระตามราคาชุดตรวจ
3. E-Coupon นี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้
4. E-Coupon นี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้
5. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น
6. ไม่รวมค่าแพทย์ในกรณีเจาะจงเลือกแพทย์เฉพาะทาง
7. ชุดตรวจสุขภาพ ไม่รวม ค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง 
8. กรุณานัดหมายล่วงหน้า 3 วันก่อนเข้ารับบริการ

📌 วันหมดอายุ E-Coupon 
1. E-Coupon จะมีอายุการใช้ตั้งแต่ 26 กุมภาพันธ์ - 31 สิงหาคม พ.ศ. 2566
2. หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้
3. ชุดตรวจสุขภาพราคานี้ สามารถสั่งซื้อได้ ตั้งแต่ 26 กุมภาพันธ์ - 31 พฤษภาคม พ.ศ. 2566
📌 หมายเหตุ
1. กรณีลูกค้ามีการขอยกเลิกคำสั่งซื้อหลังสิ้นสุด Shopee Guarantee โดยไม่ได้มีสาเหตุมาจากทางโรงพยาบาล โรงพยาบาลขอสงวนสิทธิ์ในการหักค่าดำเนินการ 10% จากราคาสุทธิในใบเสร็จ
2. ยอดเงินที่แสดงใบเสร็จเป็นยอดเงินที่ได้รับจากการชำระเงินจริงไม่รวมโปรโมชั่นส่วนลด

Suitable for adults aged 40+
Please refrain from water and food 8 - 10 hours before the service.
📌 Terms and conditions: 
1. These packages are recommended for people aged over 40.
2. Bangkok Hospital reserves the right to provide services only to patients whose E-Coupons can be presented. 
3. If E-Coupons cannot be presented on the service day, you need to pay the package fees. 
4. E-Coupons cannot be changed into cash.
5. E-Coupon is only applied to specific items as mentioned and changes cannot be made.
6. These packages are for outpatients who are physically dependent. 
7. These packages exclude doctor fees in case additional specialists are required.
8. These packages exclude treatment fees or medications prescribed by the doctors, if any. 
9. All screening items cannot be shared between patients.
10. Please make an appointment at least 3 days in advance.

📌 Validity of E-Coupons
1. E-Coupons are valid from 26 Feb – 31 Aug 2023.
2. Once E-Coupons expire, Bangkok Hospital reserves the right to neither refund nor extend service validity. 
3. Packages are available to purchase from 26 Feb – 31 May 2023.
📌 Receiving E-Coupon 
1. After payment, you will receive SMS with attached link for filling in your information in order to obtain your receipt. 
2. E-Coupon and receipt will be sent to your given Email or SMS. 
3. Your E-Coupon will be activated after you press “Order Received” 
From Shopee. Go to Me > My Purchases > To Receive > Click "Order Received”
📌Remarks: 
1. In case the customer wants to cancel purchasing order after Shopee Guarantee is over without any reasons caused by the hospital, Bangkok Hospital reserves the right to charge you 10% from total cost shown on your receipt. 
2. Receipt reflects the real amount of payment in which promotions are excluded.
